When you come for your appointment, please remember to bring the following:
- Insurance information
- Referral Letter (if required)
- Copies of results, X-rays, MRI's, CT scans etc and any other relevant information
- List of medications (if any)
We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we have kept you waiting.
To cancel an appointment
Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.
BMI policy of DNA fee being charged.
We would like to refer you to the appointment confirmation/fee letter you would have received from Circle Health when booking your appointment. As per local policy:
*our DNA/late cancellation fee: £180
*The number of hours prior to the appointment that you class as a late cancellation so will charge for e.g. 24 hours or 12 hours: 24 Hours’ Notice required
